Using a table format to run a list,

This example has 26 fighters in a double-elimination list. The scheduling takes into account the finishing order of the bouts in the previous round to help give the fighters a more equal time to rest between bouts.
[Why 26 fighters? On at least two occasions so far, the local big annual tournament has had 52 entrants. They were both run as two lists with two fields each.]

In this example, one fighter will drop out unexpectedly somewhere in the middle (as in Example 3) AND one fighter will be allowed to enter the tourney after the first round.
Close enough to real life for you? But, wait... there's more!
That's 51 total fights, more or less. Let's say we have two fields. I've ALSO randomized how long the bouts will take to show what can happen when two or three bouts get done in one field while there's a long bout in another field.
ROUND ONE
Arrange the bouts as in the previous examples.
The bouts for each field are:
Field 1: #1-#14, #2-#15, #3-#16, #4-#17, #5-#18, #6-#19, #7-#20
Field 2: #8-#21, #9-#22, #10-#23, #11-#24, #12-#25, #13-#26
And the randomized results are:

ROUND TWO
The bouts from the previous round in the order that they ended with winners listed first:
#14-#1, #8-#21, #9-#22, #10-#23, #2-#15, #16-#3, #24-#11, #17-#4, #18-#5, #19-#6,

For this example, when assigning bouts we will focus more on how long it's been since a particular fighter last fought a bout rather than trying to keep them in the same field throughout the tourney.
In the pairings for this round, we'll have at least one "winner" against one "loser" because we have 13 pairs. That's always going to happen in the table format when you have an odd number of fights in a round. From what I've heard, fighters don't have a problem with this.
The bouts for this round:
#14-#8, #1-#21, #9-#10, #22-#23, #2-#16, #15-#3, #24-#17, #11-#4, #18-#19, #5-#6, . . .
-- and that leaves three more pairs that we have to mix up somewhat to keep the last pair from fighting each other again.
We'll make those #25-#7, #12-#26, and #13-#20.
In case it comes in handy, keep in mind that #20 has fought one "mixed" bout since #20 lost the previous round but her opponent (#13) did not.
After you have all the bouts assigned, split 'em up between the two fields.

ROUND THREE
The bout finishing order from the previous round, remaining fighters only, winners first:

Pairing winners together gets interesting! Also, alternating winners and losers doesn't quite work after Round Two with several fighters dropping out each round. The pairings I'd assign are:
#14-#21, #8-#10, #9-#3, #23-#4, #2-#24, #16-#17, #5-#19, #18-#25, #12-#20, #7-#13

(however, in this case, both fighters are 1-1, so it's trivial)

So, do you remember that a fighter would be allowed to enter the tourney after the first round in this example?
By consensus of everyone involved (fighters, marshals, list keeper, spectators, and anyone else you can think of) this apparently rather popular fighter is allowed to enter as #27, but takes a loss as a penalty.
Fighter #27 will fight in the first bout of the next round.
(My personal preference on late-comers: absolutely no entrances after Round Three in a double-elimination list; Round Two for a single-elimination list.)
ROUND FOUR
Bout finishing order from previous round, winners first:
[#27], #8-#10, #21, #3, #23, #2-#24, #17, #18-#25, #13, #19, #20.
Three fighters lost in the previous round but are still in. With #27 who just "took" a loss, that makes four fighters. We can use this to arrange two bouts in this round. We'll pair #27 against #10 and #24 with #25.
We'll assign the remaining bouts as close we can to the finishing order in the previous round to give the fighters roughly the same amount of rest.

* Fighter #8 faced Fighter #21 in Round One of this list.

With just three bouts in one of the fields, I'd make a note to ask the lead marshal about going to just one field for the next round.

Now, even though #19 lost in the last round, and #13 didn't, both fighters are 3-1 in the list, so that's cool. Since we're down to four fights total, we're also going to one field (with the marshal's approval, of course).
Uh oh, late breaking news: #20 has to drop out unexpectedly.
Hmmm. Make that three fights for this round. We'll sit out #2 (the would-have-been opponent).

ROUND SIX AND ONWARD...
From previous round: [#2], #21, #8, #19.
We could assign #2-#21 and #8-#19 ... but look at their records:
#2 is 4-0 ... #8 is 5-0 ... #19 is 4-1 ... #21 is 4-1.
If we assign #2-#8 and #21-#19 instead, someone definitely drops out.
Also, that way we'll have a 4th place finisher.

So, Fighter #21 drops out with two losses and earns 4th place with a respectable record of 4-2.
OK, Fighter #2 is 4-1 ... #8 is 6-0 ... and #19 is 5-1.
It's easy. We assign #2-#19 since they both have one loss each.
Optionally, we could ask #19 about having #2 fight an extra bout beforehand, say against #21, to even things up (six bouts each).
Either way, the winner of #2-#19 (when ready) faces #8 for the final.
Total number of bouts before the final: 49.
With a two-out-of-three final... you got it, 51.
Well, how about that... Even with someone coming in late and someone else dropping out.
These four examples have been fake; the results all randomized.
